zennya, Inc. (“zennya”) has created this privacy policy in order to explain our privacy practices. The following discloses our information-gathering and dissemination practices for the website owned and operated by zennya (the “Site”), services and the zennya mobile application (collectively, the “Service”).
By using the Service, you do hereby warrant and represent that you have read, understand and agree to all terms and conditions stated herein, including our Terms of Use, available at http://www.zennya.com/terms, which is incorporated herein by reference, which govern the use of the services offered by zennya, and that you are giving zennya permission to use and store such information consistent with this privacy policy and our Terms of Service.
A user of the Service is hereinafter referred to as a “User,” or collectively, the “Users.”

This notice describes the privacy practices of the independent professional health care entities that contract with zennya, Inc., operate as affiliated covered entities, and provide the requested health care services, including Remedium Inc. HDSI, Reliance, Inc., and other practices with which zennya, Inc. may contract in the future (collectively, the “Practices”). Among other things, the Practices contract with zennya, Inc. to license the zennya mobile application to provide the Practices with the capability to receive, respond to and schedule requested visits. zennya, Inc. does not provide any medical services, nor does it refer or recommend any physician or medical practice or any other health care provider or personnel.
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.
Get an electronic or paper copy of your medical record: You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this. We will provide a copy or summary of your health information in a time frame compliant with the law, usually within 15 days of your request. We may charge a reasonable, cost-based fee.
Ask us to correct your medical record: You can ask us to correct health information about you that you think is incorrect or incomplete. Ask us how to do this. We may say “no” to your request, but we’ll tell you why in writing within 60 days.
Request confidential communications: You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests.
Ask us to limit what we use or share: You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care. If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purposes of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
Get a list of those with whom we’ve shared information: You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why. We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.
Get a copy of this privacy notice: You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
Choose someone to act for you: If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority and can act for you before we take any action.
File a complaint if you feel your rights are violated: You can complain if you feel we have violated your rights by contacting us using the information below. We will not retaliate against you for filing a complaint.
For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.
In these cases, you have both the right and choice to tell us to: (1) Share information with your family, close friends, or others involved in your care; (2) Share information in a disaster relief situation. If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety.
In these cases, we never share your information unless you give us written permission: marketing purposes, sale of your information, and most sharing of psychotherapy notes.
In the case of fundraising: We may contact you for fundraising efforts, but you can tell us not to contact you again.
In the case of information obtained from Apple’s Clinical Health Records API or through synchronization with the Apple Health application: We do not disclose any data obtained through Apple’s Clinical Health Records API or through synchronization with the Apple Health application to any third-party for advertising, marketing, or other use-based data mining purposes, other than improving health management and/or treatment.
OUR USES AND DISCLOSURES
We typically use or share your health information in the following ways:
Treat you: We can use your health information and share it with other professionals who are treating you. For example, a doctor treating you for an injury asks another doctor about your overall condition.
Run our organization: We can use and share your health information to run our practice, improve your care, and contact you when necessary. For example, we use health information about you to manage your treatment and services.
Bill for your services: We can use and share your health information to bill and get payment. For example, we give information about you to your health insurance plan so it will pay for your services.
How else can we use or share your health information?
We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. We have to meet many conditions in the law before we can share your information for these purposes.
Help with public health and safety issues: We can share health information about you for certain situations such as (1) preventing disease; (2) helping with product recalls; (3) reporting adverse reactions to medications; (4) reporting suspected abuse, neglect or domestic violence; or (5) preventing or reducing a serious threat to anyone’s health or safety.
Do research: We can use or share your information for health research.
Comply with the law: We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we’re complying with federal privacy law.
Respond to organ and tissue donation requests: We can share health information about you with organ procurement organizations.
Work with a medical examiner or funeral director: We can share health information with a coroner, medical examiner, or funeral director when an individual dies.
Address workers’ compensation, law enforcement, mandated reporting and other government requests: We may use or share health information about you: (1) for workers’ compensation claims; (2) for law enforcement purposes or with a law enforcement official; (3) with health oversight agencies for activities authorized by law; (4) for special government functions such as military, national security, and presidential protective services; (5) for any applicable mandated reporting purposes such as child abuse, sexual assault, intimate partner violence or other mandated reporting.
Respond to lawsuits and legal actions: We can share health information about you in response to a court or administrative order, or in response to a subpoena.
In many circumstances, we are required to provide more restrictive treatment to the following types of information: psychotherapy notes or other behavioral health information, genetic testing information, information on persons with developmental disabilities, information concerning HIV/AIDS testing, and alcohol and drug abuse treatment.
We are required by law to maintain the privacy and security of your protected health information. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We must follow the duties and privacy practices described in this notice and give you a copy of it. We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind.
Personal Information. zennya collects personal information only if you voluntarily choose to share such information with zennya. You may use portions of the app without disclosing any personal information. However, in order for you to take advantage of particular opportunities provided through the Service, we may require that you furnish personal information. When you register your account, sign up for newsletters, complete forms for optional programs, surveys, contests and other entries, and through communications transmitted through zennya, applications and your use of the zennya mobile application, we collect information including your name, address, phone number and email address. You also may choose to send zennya personally identifying information in an email message containing information or inquiries about the Service.
Anonymous or Aggregated Data. zennya collects and stores certain data about the use of the Service on an aggregate and/or anonymous basis. zennya does not link this information to anything personally identifiable to you.
Cookies and Other Technologies. When you visit websites on the Internet, your browser may automatically transmit information to the websites you visit throughout every visit. Like many websites, we may use “cookies” to collect information. A cookie is a small data file that we transfer to your computer’s hard disk for record-keeping purposes. We may utilize persistent cookies that only zennya can read and use to save your login information for future logins to the Service, to assist in processing items in your shopping basket, to hold session information, and to track user preferences. We may utilize session ID cookies to enable certain features of the Service, to better understand how you interact with the Site and to monitor aggregate usage by our users and web traffic routing on the Service, to track the number of entries in zennya promotions, sweepstakes and contests and to identify visited areas of the Site and Service. Unlike persistent cookies, session cookies are deleted from your computer when you log off from the Site and Service and then close your browser. Third party advertisers on the Site may also place or read cookies on your browser and we may use Flash cookies (or local shared objects) to store your preferences or display content based upon what you view on our site to personalize your visit. You can instruct your browser, by changing its options, to stop accepting cookies or to prompt you before accepting a cookie from the websites you visit. If you do not accept cookies, however, you will not be able to use all portions or all functionalities of the Service. We and our service providers may also use “pixel tags,” “web beacons,” “clear GIFs,” or similar means in connection with the Service and HTML-formatted email messages to, among other things, track the actions of Users and email recipients, to determine the success of marketing campaigns and to compile aggregate statistics about Site usage and response rates. We may also engage one or more third party service providers to serve online advertisements on our behalf. They may use a “pixel tag” to collect anonymous information about your visits to this Site and to other websites, and they may use that information to target advertisements for goods and services. This information is collected anonymously, in a manner that does not personally identify you.
IP Address. When you visit and interact with the Site, zennya and/or third parties with whom zennya has contracted to provide services to zennya may collect your Internet Protocol Address (IP Address, which is a number that is automatically assigned to the computer that you are using by your Internet Service provider). This number is identified and logged automatically in our server log files whenever you visit the Site, along with the time(s) of your visit(s) and the page(s) that you visited. We use IP Addresses to monitor the regions from which Users navigate the Site and for fraud prevention purposes.
The Company also collects and uses personal data regarding its employees (“Employee Data”), which may include: name, address, job title and other job information, location, compensation information, identification number, employment history, and a copy of employment agreements.
We may also require you to provide us with your financial information, including billing name, address and credit card number (“Financial Information”).
Only authorized zennya staff have access to your Personal Information in its entirety. Other staff may have access to portions of this information if it pertains to their job duties.
zennya uses Personal Information for the following general purposes:
Although we ensure that our third party agents (“Agents”) receiving your information are bound by privacy restrictions at least as restrictive as those set forth in this policy, zennya is not responsible for any issues that may arise regarding the Privacy Policies of any of its affiliates or advertiser. By using this website, you do hereby agree to hold zennya harmless from any claims and/or damages that may arise from the actions of any of its affiliates and/or advertisers.
Two things drive zennya’s decisions regarding the use of information collected from Users: 1) improving user experience by offering personalized services, content and advertising, and 2) market research.
Ads are a standard part of user experience on the Internet, and zennya believes that targeted advertising enhances this experience. zennya may use Cookies and other Technologies and Personal Information to place ads where they believe interested parties will see them.
In addition to banner ads, zennya may advertise products, companies and events that we think might interest you through the email address you provide.
zennya may analyze Personal Information and Web Navigational Data based on your interests and may use the analysis from this information in a way that doesn’t reveal a User’s personal information (“Market Research”) in order to:
zennya uses data at the individual level to sell products and services to Users who express an interest in these products and services, through a poll for example, or to Users who can be presumed to have an interest based on results from our Market Research.

Employee Data is only used for the for the purposes of facilitating the performance of certain administrative tasks and functions relating to general employment and for processing and investigating reports under the Company’s codes and policies ( “Employee Data Purposes”).
We use Financial Information solely as authorized by you in accordance with the Service.
You may opt-out of receiving communications from us and our partners, remove your information from our database, and choose to not receive future communications unrelated to the Service.
During registration you choose whether to receive correspondence from either zennya or its partners and whether you want to participate in a variety of zennya programs. This information remains on your Profile where you can, at any point, easily edit it to indicate that you have changed your mind. After logging on, click on your profile on the top right of any page, then select “Notifications.” You may then click on “email alerts,” and make your selections. If you purchase anything through zennya, we may occasionally contact you with information about special events, sales, activities, promotions, contests, submission opportunities and programs. You always have the option to ask zennya not to contact you with this information again.

To preserve the integrity of our databases, we may retain information submitted by Users for an indefinite length of time. If required by law, we will delete Personal Information by erasing it from our database. We will also respond to User requests to delete or correct account or Personal Information, which you can do by contacting zennya at delete@zennya.com, or by selecting the “Notifications” option under your account icon in the top right corner of your home page or on your device.
Videos and Images of zennya
For each User seeking to provide task services via the Service (each, a “Tasker”) who has provided zennya with any videotape, film, record, photograph, voice, or all related instrumental, musical, or other sound effects, you consent to our use of these materials in accordance with Section 13 of the Terms of Use.
Your zennya account is password-protected so that only you and authorized zennya staff have access to your account information. In order to maintain this protection, do not give your password to anyone. If someone who represents to you they are zennya staff and asks for any personal account information, including password, check their URL. If it doesn’t say they are help@zennya, they aren’t. Also, if you share a computer, you should sign out of your zennya account and close the browser window before someone else logs on. This will help protect your information entered on public terminals from disclosure to third parties.
zennya makes every effort to ensure that your information is secure on its system by backing up data in separate back up drives and uses public key access encryption and firewalls on both our main and backup servers. We also employ and customer record managers to protect against loss, misuse and alteration of the information under our control. Extremely sensitive information, like credit card numbers, is always transmitted in encrypted form. Only certain authorized zennya staff can access Personal Information, and then only if it is relevant to their job duties. zennya has staff dedicated to maintaining this Privacy Policy and other privacy initiatives, periodically reviewing security and making sure that every zennya employee is aware of our security practices. Unfortunately, no data transmission over the Internet can be guaranteed to be 100% secure. As a result, zennya cannot guarantee the security of any information you transmit to us, and you do so at your own risk. While zennya will use commercially reasonable efforts to ensure the security of all credit card and all other personal information, we expressly disclaim any liability for any unauthorized access to or use of our secure servers and/or any and all personal and/or financial information stored therein, and you agree to hold zennya harmless for any damages that may result therefrom. If you have any further questions on this issue, please refer to the Terms of Use. zennya expressly disclaims any liability that may arise should any other individuals obtain the information you submit to this website.
When you submit information to zennya, it is collected, processed and maintained solely by us or by third party agents who are bound by privacy restrictions at least as restrictive as those set forth in this policy. We may allow our business partners to collect information about Users that is not extremely sensitive Information. If you would like to opt out from the onward transfer of your information to our non-agent partners, you may send us an email at help@zennya.com. Include the word “privacy” in the subject line of the email and all your registration information in the body of the email.
Some zennya content is “sponsored by” or “presented by” other companies. zennya shares market research data (how the sponsorship performed, names of contest winners, etc.) collected on this website or via email surveys with these partners. This information is aggregate and not linked to specific users.
Partners may sponsor promotions, contests and sweepstakes that require you to reveal details such as your name, address or phone number. Always check their privacy policy before revealing information about yourself. If you don’t want these sponsors to have your personal information, you can choose to not participate.
zennya reserves the right to disclose Personal Information from both private and public areas of this website at our discretion, if required by law or if we are given reason to believe, in our sole discretion, that someone is causing injury to or interference with the rights of Users, the general public, or zennya, to comply with a judicial proceeding, court order or legal process.
We will not transfer to a non-Agent third party any Personal Information that is sensitive in nature, such as information specifying your medical or health condition, racial or ethnic origin, political opinions, religious or philosophical beliefs, trade union membership without your prior affirmative consent.
Finally, zennya reserves the right to transfer, sell and/or distribute all information collected through the Service to an affiliate, subsidiary, or third party in the event of any reorganization, merger, sale, joint venture, assignment, transfer or other disposition of all or part of our business, assets or stock. We will use our best efforts to ensure that the new entity follows this Privacy Policy with respect to your Personal Information, as and to the extent required by applicable law.
zennya believes strongly in giving you the ability to access and edit your personal information. To update your personal info, click My Account or Login at the top of any page. There you can view, update and correct your account information. You may edit your account information at any time — all you need is your username and password.
So that we can protect the integrity of sensitive data, there are certain pieces of information, such as your age, that you cannot alter yourself. For example, since children under 18 are not allowed to register as Users, we need to take reasonable measures to preserve the accuracy of our Users’ ages. Our databases automatically update any Personal Information you edit in your profile, or that you request we edit. Information transmitted through boards, chats, polls or through any other means remains in our databases and becomes the property of zennya upon submission.
zennya’s security and privacy policy are periodically reviewed and enhanced as necessary. This policy might change as zennya updates and expands the Service. zennya will endeavor to notify you of these changes by email, but will not be liable for any failure to do so. zennya also encourages you to review this privacy policy periodically.
